  • Employees can go in or out of network as they wish but will realize greater savings by visiting in-network providers.

(HMO) Health Maintenance Organization Plans

Most affordable option, with an established network of doctors.

  • Members may need to choose their primary care physician, and referrals are obtained for specialists.

(POS) Point of Service Plans

A hybrid of HMO and PPO

  • Like an HMO, participants designate an in-network primary care physician.

  • But like a PPO, patients may go outside of the provider network for services.

  • Coverage amounts are reduced when using out of network options.

(HSA) Health Savings Account Plans

Accounts are used with a qualifying high deductible health plan (HDHP).

  • Employee accounts are funded on a pre-tax basis.

  • They are used to pay qualified medical expenses not paid by the high deductible plan, including office visits, deductibles, co -insurance and prescriptions.

  • The account is owned by the employee.
